StarPoint provides a full range of disease condition management programs that were developed with Milliman USA guidelines. StarPoint’s disease management programs have disease-specific assessments, complete case management care plans (goals, interventions, outcomes), and educational materials for program participants. Predictive modeling software is used to identify members with a particular condition.  Disease conditions that we manage include:

Wellness Programs

Wellness programs target the total population in a plan using Predictive modeling software. However, participation is not primarily driven by a disease. Wellness programs are designed to help individuals maintain and improve health and well-being, increase awareness of factors that can affect health and longevity; enable individuals to take greater responsibility for their health behaviors; prevent or delay the onset of disease; and promote healthful lifestyles and general well-being.
